Who we are

Create Wellness Center (business registration 322-88-02866) operates two clinics: Hannam Bldg 1st Fl, 737-37 Hannam-dong (Itaewon-ro), Yongsan-gu, Seoul 04349, and Global Edu Ro 390, Bldg B, Unit 115, Daejeong-eup, Seogwipo-si, Jeju. What we collect, and why

We collect only what we need to book you in, treat you safely and handle your paperwork.

• Contact and identity details — name, date of birth, email, phone number. Used to create and maintain your clinic record, confirm appointments and send reminders.
• Insurance details — insurer, member number, group or employer name, and a photo of your insurance card if you upload one. Used to bill TSS directly or to submit a claim to your insurer when you ask us to, and to prepare your PDF invoices and claim documents.
• Health information — the symptoms, history and lifestyle details you share on the intake form and during visits, and the notes your practitioners keep. This is sensitive information under Korea's Personal Information Protection Act, so we ask for your separate, explicit consent before collecting it. Used only for your assessment and care.
• Booking-request and contact messages — whatever you type into the booking-request form or send us by email. Used to answer you.
• Optional marketing consent — if you tick the box, occasional clinic news by email. You can unsubscribe at any time and it never affects your care.

We do not use cookies for tracking or advertising, and this website contains no third-party analytics or ad scripts.

Who sees it

• Our clinic team — your treating practitioners and reception staff, who are bound by confidentiality.
• Your insurer — only when you ask us to bill TSS directly or to submit a claim, and only the details the claim requires.
• Service providers that store or deliver data for us — the online intake form is processed and stored through Supabase; the booking-request form is delivered through Netlify, which also hosts this website; appointment scheduling runs on Mindbody (Seoul) and Acuity Scheduling (Jeju). Each acts on our instructions and does not use your information for its own purposes.

We never sell your information and never share it for anyone else's marketing. Some of the providers above store data outside Korea; where they do, we rely on their contractual commitments to protect it to the standard Korean law requires.

How long we keep it

For as long as you remain a client, and afterwards only for the period Korean law requires clinics to keep treatment records, after which it is securely deleted. Booking-request and contact messages that do not lead to a visit are deleted within one year. Marketing consent lasts until you withdraw it.

Your rights

You can ask us at any time to see the information we hold about you, correct it, delete it, or pause its use, and you can withdraw any consent you have given. Email info@createwellnesscenter.com and we will respond within ten working days. Withdrawing consent for health information may mean we can no longer treat you safely, and we will tell you if that is the case. If you are not satisfied with our answer you may contact the Personal Information Protection Commission (privacy.go.kr, 국번 없이 182).

How we protect it

Online forms are sent over encrypted connections and stored in access-controlled systems that only named clinic staff can open. Paper records are kept in locked storage at the clinic. Staff access is limited to what each role needs.

Children

Where the client is under 14, we collect information from and with the consent of a parent or legal guardian, who completes the intake form on the child's behalf.

Changes to this policy

If we change how we handle your information, we will update this page and the effective date above. Significant changes will also be posted at reception.
